Job summary

Extended Care, LLC is seeking a dedicated Activity Aide to elevate the daily living experience for our residents through engaging activities with staff and fellow residents. You will facilitate ongoing group and individual programs based on assessments and care plans, while completing required records for admissions, discharges, and transfers.

• 1-2 years of experience in a social or recreational program in a healthcare setting (preferred).

Responsibilities

• Facilitate ongoing group and individual activity programs based on residents’ needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events.
• Follow and maintain established policies and procedures.
• Encourage resident participation in Resident Council and schedule monthly meetings.

Activity Aide Requirements:
