Job summary

Clarity Care in Wisconsin is hiring an Activity Support Provider to help with the Day Services Program, supporting members in community activities and daily living tasks. You will assist with personal care, meals, and goal-oriented activities tailored to each member while ensuring safety and respect.

No prior experience is required; starting pay is up to $18/hr based on experience, with benefits and opportunities for growth within a mission-driven nonprofit.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards.
• Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including DOJ/other state checks.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision-making.
• Create fun and engaging activities tailored to member interests.
• Prepare and serve meals following dietary needs and preferences.
