Clarity Care, Inc. is seeking an Activity Support Provider to assist the Day Services Program in engaging members in community and recreational activities, as well as daily living tasks. The role emphasizes supportive, person-centered care with opportunities for growth.

Starting pay up to $18/hr based on experience, with weekend shift premium, paid training, and comprehensive benefits. No prior experience or certifications required, and we’re hiring immediately to help people in our Wisconsin

Qualifications

• Must possess a high school diploma/equivalent.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision‑making.
• Create fun and engaging activities tailored to member interests.
• Prepare and serve meals following dietary needs and preferences.

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.
