Clarity Care is seeking a Personal Care Worker to assist Members with activities of daily living, including bathing, grooming, meal preparation, and light housekeeping.

Qualifications

• Must be a certified Personal Care Worker or have prior personal care experience.
• High school diploma/equivalent required; 18+ years old.
• Valid WI driver’s license and ability to meet driving standards.

Responsibilities

• Provide hands-on personal care, bathing, grooming, toileting and hygiene.
• Perform housekeeping, laundry, meal prep and medication reminders as needed.
• Document tasks accurately and maintain a safe, clean home environment.
• Offer companionship and assist with errands and transportation as required.

Provides support to our Members by assisting with activities of daily living; bathing, grooming, toileting, and hygiene. Be comfortable using mobility devices and practice safe transfer and ambulation techniques.Completes all applicable documentation related to tasks assigned.Welcoming Home.
