Clarity Care Inc. is hiring an Activity Support Provider to assist individuals in our Day Services Program in Oshkosh, WI. You will help with daily living activities, accompany members in community events, and support their goals in a respectful, person-centered way.

No prior experience or certifications are required to apply; training is provided. This on-site role offers competitive wages and a chance to impact lives in your local community.

• Valid Wisconsin driver’s license and valid auto insurance proof.
• Must pass a Wisconsin caregiver background check (and previous states if applicable).

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.

Education/Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
