Job summary

AltaMed Health Services Corp. is seeking an Activities Coordinator to join the PACE Interdisciplinary team. You will participate in assessments, care planning, and design engaging activities tailored to physical, cognitive, social and spiritual well-being of PACE participants.

You may use arts, music, movement, and community outings to foster a vibrant, supportive environment. You will adhere to PACE program guidelines, work under supervision of the PACE Care Center Supervisor, and contribute to

Qualifications

• Minimum of a High School diploma or equivalent; a bachelor’s degree is highly desirable.
• Minimum of 1 year of experience in a social or recreational program for frail or elderly population; dementia care experience preferred.
• Completion of 36 hours Activity Leader course approved by the State of California within 30 days of hire.

Responsibilities

• Participate in care coordination activities within the PACE IDT.
• Design and oversee a variety of engaging activities (arts, crafts, music, movement, community outings).
• Ensure activities promote physical, cognitive, social, and spiritual well-being; may use remote technology when appropriate.
