Participate in planning and conducting individual, small, and large group activities. Assist in facilitating communication between employees at all levels, residents, their families, support personnel, government agencies, and the public to meet the needs and interests of residents, the community, and the facility. Contribute to community planning aligned with the facility's interests and the needs of residents and their families.
Assist in developing the monthly activity calendar and maintaining attendance records. Support the Quality Assurance and Assessment Committee in creating and implementing plans to address quality deficiencies as directed by the Activities Director. Participate in discharge planning, and help develop and implement activity care plans and resident assessments.
Assist in arranging transportation for residents for outings, appointments, or discharges. Develop a monthly activity schedule that includes outings, group activities, and in-room activities for bed-bound or isolated residents. Encourage residents to participate in self-initiated activities such as hobbies, crafts, and reading, providing materials like Braille or audiobooks as needed.
Assist with documentation, including quarterly progress notes, as assigned by the Activities Director. Maintain the Activity Department in a clean, orderly, and secure condition.
This position has no supervisory responsibilities.
Requires standing, walking, reaching, pushing, pulling frequently; sitting occasionally; talking, hearing, tasting, and smelling frequently; lifting up to 50 pounds frequently; and occasional climbing, stooping, kneeling, crouching, or crawling. Vision abilities include close, distance, color, peripheral, and depth perception, and focus adjustment.
The environment is typically low to moderate noise. Reasonable accommodations are available for individuals with disabilities. Management may reassign duties as needed, and responsibilities may evolve.
