Position Overview
Polaris Rehabilitation and Care Center is seeking a compassionate, energetic, and resident-focused Activities Assistant to join our Activities Department. This role works closely with the Activities Director to plan, organize, and facilitate engaging programs and events that promote residents' physical, emotional, social, and cognitive well-being. The Activities Assistant plays a vital role in enhancing residents' quality of life by encouraging participation, fostering meaningful connections, and creating a positive, enriching environment throughout the facility.
Key Responsibilities
- Assist the Activities Director in planning, organizing, and implementing daily activity programs for residents
- Encourage and assist resident participation in group activities, recreational programs, special events, and community outings
- Support residents during activities by providing assistance, encouragement, and supervision as needed
- Help prepare activity materials, supplies, decorations, and equipment for scheduled programs and events
- Maintain accurate attendance records and documentation related to resident participation
- Assist with resident room visits and one-on-one activities for residents unable to participate in group programming
- Transport residents safely to and from activities within the facility when needed
- Observe and report changes in resident mood, behavior, interests, or participation levels to the Activities Director and nursing staff
- Help create a welcoming, engaging, and inclusive environment that promotes resident independence and dignity
- Participate in holiday celebrations, special events, and facility-wide activities
- Maintain activity areas in a clean, organized, and safe condition
- Adhere to all facility policies, safety procedures, infection control standards, and applicable state and federal regulations
Qualifications
- High school diploma or equivalent required
- Previous experience working with seniors, long-term care residents, or recreational activities preferred but not required
- Ability to communicate effectively and professionally with residents, families, and team members
- Strong organizational and interpersonal skills
- Positive attitude, creativity, and enthusiasm for working with older adults
- Ability to work independently while collaborating effectively within a team environment
- Ability to perform essential job functions with or without reasonable accommodation

Physical Requirements
- Ability to assist residents with mobility and transportation throughout the facility
- Ability to stand, walk, bend, lift, and perform physical tasks for extended periods
- Ability to lift and carry activity supplies and equipment (up to 25 lbs. regularly and more with assistance as needed)
- Ability to respond promptly and effectively in emergency situations
