The role requires NA-C certification preferred; day shift, 80 hours per pay period. Responsibilities include planning and documenting assessments in EPIC, engaging residents, and coordinating with staff while maintaining dignity and respect.

Qualifications

• Must have NA-C certification or be eligible for HCA licensure.
• High school diploma or equivalent required.

Responsibilities

• Provide direction for recreational, social, and spiritual activities for residents.
• Notify licensed staff when a patient reports pain.
• Interview and complete Activity Assessments within 14 days of admission.
• Interview and assess Swing Bed patients and document care plans in EPIC.
• Maintain activity programs to support residents' physical, mental, and social functioning.
• Treat residents and families with respect, addressing psychosocial and spiritual needs.
• Assist with voter registration and address changes as needed.
• Assist in providing and leading activities for residents who cannot leave their rooms.
• Oversee maintenance of activity equipment and supplies.
• Engage residents with compassion and effective communication.

Primary Scheduled Hours: Day shift, 80 hours per pay period

Job Summary: Is responsible for assisting, leading, and overseeing a diverse range of recreational activities to engage participants, ensuring their safety and overall enjoyment while adhering to established guidelines and budget constraints.

Essential Job Functions:

• Provides direction for recreational, social, and spiritual activities for the facility residents.
• Notifies appropriate licensed staff when patient complains of pain.
• Interviews and completes Activity Assessments within 14 days of admission.
• Interviews and completes assessment of Swing bed patient upon notification. Completes care-plans and documents into EPIC.
• Maintains programs designed to make life more meaningful, to fully stimulate and support physical and mental capabilities and to enable the resident to maintain the highest attainable social, physical and emotional functioning.
• Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of residents and their families.
• Assists with voter registration and address changes as needed.
• Assist in providing and leading activities for residents unable to leave their rooms.
• Oversees maintenance of activity equipment and supplies.
• Assists residents with activities; is compassionate, sensitive to other’s needs, and understanding. Motivates, listens to, and converses well with older people.

Education Background and Experience Requirements

High School graduate or equivalent.

Required Licensure / Certifications:

NA-C preferred but may have HCA

Required Experience:

NA-C experience and or experience in a Residential Care setting.

Preferred Qualifications:

DD, Mental Health, and Dementia Training.

Food Handler’s Card.

Successful candidates typically begin between the beginning and midpoint of the compensation range dependent on qualifications/experience and receive compensation in increments increases annually.
