CNS Healthcare is seeking an Assertive Community Treatment (ACT) Team Supervisor to oversee daily ACT operations, supervise staff, ensure contract and MDHHS compliance, and promote high-quality community-based care. The role requires QMHP with appropriate licensure and 2+ years of post-degree experience with adults with serious mental illness.

Qualifications

• QMHP with required degree and licensure.
• 2+ years post-degree clinical experience with adults who have serious mental illness.
• Knowledge of HIPAA in health care.
• Participation in ACT-specific training within 6 months of hire and annually thereafter.
• Ability to understand and value cultural diversity.

Responsabilités

• Provide day-to-day supervision of the ACT team and model professional behavior.
• Ensure ACT requirements in contracts and MDHHS Manual are met, with regular team meetings.
• Aim for 80% ACT contacts in beneficiaries' homes or community locations.
• Support team in service planning, treatment and community-based direct services.
• Maintain productivity and documentation standards; complete reporting on time.
• Ensure staff trainings are attended and documented.
• Promote quality improvement and collaboration with other departments.
