Job summary

Seven Counties Services is seeking a clinical leader to direct the ACT team, oversee day-to-day operations, and supervise staff. The role emphasizes treatment planning, admission processes, and coordination of care for individuals with severe mental illness in community settings.

The ideal candidate will have a Master’s degree in Social Work or Psychology and at least six years of clinical experience, including supervisory roles. Valid driver’s license and full-time availability are required.

Responsibilities

• Direct day-to-day clinical operations of ACT team.
• Provide direct supervision of team members.
• Schedule staff work hours to ensure appropriate coverage.
• Lead daily team and treatment planning meetings.
• Evaluate status of all consumers and provide appropriate coordination of treatment to meet their challenging needs.
• Direct and coordinate consumer admission and assessment processes.
• Schedule admission interview.
• Supervise, direct, and coordinate completion of comprehensive assessment/reassessment of each consumer.
• Utilize management tools to ensure ACT model alignment with team goals.
• Participate in staff recruitment, interviewing, hiring, orientation, and performance plans.
• Oversee medical record documentation to ensure quality and accuracy.
• Provide on-call crisis interventions and backup as per policy.
• Engage and motivate clients in treatment goals while providing direct service.
• Manage ACT Team to maintain productivity standards.
• Oversee completion of all required documentation timely.

Job Description: Under administrative direction, provides clinical leadership to the ACT Team in assessment, treatment planning, general healthcare, medical and psychosocial approaches. Assertive Community Treatment (ACT) is designed to engage persons with a severe mental illness (SMI) whose needs are not met by traditional outpatient services. Services are delivered in the context and environment where they are needed (i.e., the team goes to the person).
