Job summary

Elizabeth Layton Center seeks an RN to join its Assertive Community Treatment (ACT) team in Ottawa, KS. The RN will deliver nursing services in the community and at the center, focusing on medication administration, health education, and integrated care for adults with complex needs.

Under supervision of the ACT Team Coordinator, the RN provides assessments and collaborates with physicians, pharmacies, and other agencies to promote recovery and reduce hospital utilization.

Qualifications

• Must have completed an accredited nursing program and be registered with the State of Kansas.
• Must possess a valid driver’s license and reliable transportation.
• Able to pass all required background checks (e.g., KBIs, NCIC, etc.).

Responsibilities

• Provide nursing assessments for ACT clients as part of the treatment team.
• Administer medications and injections and provide coaching on medications.
• Coordinate with physicians, hospitals, pharmacies, and other agencies.
• Provide health education and wellness coaching to clients.
• Maintain accurate clinical documentation per guidelines and policies.
• Support ACT team with nursing perspective during meetings and in the community.
