Job summary

Wyandot Behavioral Health Network is seeking an LPN to join the Assertive Community Treatment (ACT) Team. The role focuses on nursing care, care coordination, and support for adults with serious mental illness in community settings.

You'll collaborate with a multidisciplinary team, provide home visits, medication education, crisis support, and documentation, while traveling across the community to meet clients where they live and participate in team meetings to develop individualized treatment

Qualifications

• Current Kansas Licensed Practical Nurse (LPN) license in good standing.
• Valid driver's license with a clean driving record.
• Current automobile insurance.
• Reliable transportation and willingness to travel throughout the community.
• Ability to pass a vehicle inspection or provide proof of inspection completed within the past year.

Responsibilities

• Provide nursing care in clients' homes and other community settings, including nursing assessments, medication administration, education, and health monitoring.
• Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners for continuity of care.
• Advocate for clients to access medical, behavioral health, and community resources.
• Educate clients and families about medications and wellness strategies.
• Assist with crisis intervention and emergency response when needed.
• Participate as an active member of the ACT team and contribute to individualized treatment plans.
• Help clients develop skills to improve independence and manage emotional distress.
• Build trusting, recovery-focused relationships with clients and families.
• Complete documentation accurately and in a timely manner.
• Transport clients and provide services throughout the community.
