Wyandot, Inc. is seeking a compassionate RN or LPN to join our Assertive Community Treatment (ACT) Team, delivering evidence-based nursing and care coordination in community settings.

You will work with a multidisciplinary team to support adults with serious mental illness, focusing on recovery, independence, and connection to care. Responsibilities include in-home nursing, health monitoring, medication administration and education, care coordination with psychiatrists and other providers,

Qualifications

  • Current Kansas LPN license in good standing.
  • Valid driver's license with a clean driving record.
  • Current automobile insurance.
  • Reliable transportation and willingness to travel throughout the community.
  • Ability to pass a vehicle inspection or provide proof of inspection completed within the past year.

Responsibilities

  • Provide nursing care in clients' homes and other community settings, including nursing assessments and medication administration.
  • Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners to ensure continuity of care.
  • Advocate for clients by helping them access medical, behavioral health, and community resources.
  • Educate clients and families about medications, physical health, mental health conditions, and wellness strategies.
  • Assist with crisis intervention and emergency response when needed by coordinating with crisis services and medical providers.
  • Participate as an active member of the ACT team and help develop individualized treatment plans.

What You'll Do

As an ACT Team LPN, you'll provide nursing care, care coordination, and support to adults receiving community‑based behavioral health services. Working alongside therapists, case managers, peer support specialists, prescribers, and other professionals, you'll help clients remain healthy, stable, and connected to the care they need.

Primary Responsibilities
  • Provide nursing care in clients' homes and other community settings, including:
    • Nursing assessments
    • Medication administration
    • Medication education
    • Health monitoring
  • Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners to ensure continuity of care.
  • Serve as an advocate for clients by helping them access medical, behavioral health, and community resources.
  • Educate clients and their families about medications, physical health, mental health conditions, and wellness strategies.
  • Assist with crisis intervention and emergency response when needed by coordinating with crisis services, emergency responders, supervisors, and medical providers.
  • Participate as an active member of the ACT team, attending team meetings and collaborating to develop individualized treatment plans.
  • Help clients develop skills to improve independence and manage emotional distress.
  • Build trusting, recovery‑focused relationships with clients and their families.
  • Complete documentation accurately and in a timely manner.
  • Transport clients and provide services throughout the community as needed.

Requirements

Required Qualifications
  • Current Kansas Licensed Practical Nurse (LPN) license in good standing.

Preferred Qualifications
  • Experience working in behavioral health, community mental health, or psychiatric nursing.
  • Experience supporting individuals with severe and persistent mental illness (SPMI).
  • Experience working with individuals who have co‑occurring mental health and substance use disorders.
  • Knowledge of community‑based services, care coordination, and evidence‑based behavioral health practices.
