Job summary

Copa Health seeks a Team Specialist for our Assertive Community Treatment (ACT) team to deliver community-based behavioral health services to individuals with Serious Mental Illness (SMI). The role emphasizes recovery-oriented, person-centered support, primarily in members’ homes and community settings.

The position includes after-hours coverage and collaboration with a multidisciplinary team to promote stability, independence, and individualized treatment goals.

Qualifications

• Bachelor’s degree or equivalent with six years of education/experience and at least 2 years direct experience with Serious Mental Illness (SMI) or co-occurring disorders.
• ACT Team experience preferred.
• Bilingual English/Spanish preferred.
• Fingerprint clearance card issued by AZ DPS or eligible within 8 weeks of hire.
• Reliable transportation and AZ Driver’s License with state minimum insurance.
• TB Test clearance at time of hire.

Responsibilities

• Provide community-based behavioral health services and supports to individuals with SMI in homes, shelters, hospitals, and other settings.
• Collaborate with a multidisciplinary ACT Team to promote recovery and independence.
• Assess member strengths, needs, and progress; participate in developing and reviewing treatment and crisis plans.
• Provide rehabilitation, skill development, counseling, coaching, and recovery support to improve functioning.
• Help members access behavioral health, medical, housing, employment, education, and other resources.

Education

Bachelor’s degree in Behavioral Health or related field

Six years total education/experience including 2+ years with SMI or co-occurring disorders
