Qualifications

• Bachelor’s degree in a behavioral health related field
• Associate’s degree in a behavioral health related field with one year of full-time work experience providing behavioral health services
• High School Diploma or GED and three years’ experience providing behavioral health services to adult mental health, seriously mentally ill, substance use or children
• Valid AZ driver’s license and proof of insurance
• Arizona fingerprint clearance
• At least 21 yrs. of age
• Current CPR and First Aid certification
• Experience working with clients with complex emotional and functional needs
• Bachelor’s degree in Behavioral Health Field with at least one year behavioral health experience
• Knowledge of community resources available, including benefits and entitlement process, as well as an understanding of managed care
• Understanding of client/family-centered and ACT models of care

Responsibilities

• Provide clinical expertise and oversight of client care and coordinate with ACT team members
• Assess clients through ongoing assessments and risk evaluations
• Engage with clients and families to build trust and engagement in services
• Assist in development of Adult Treatment Team and coordinate care
• Complete additional assessments to monitor progress
• Assist in safety/crisis plan creation and triage emergencies
• Develop and monitor Behavioral Health Service Plans (BHSP)
• Coordinate care activities and staffings with ACT team members
• Assist clients/families in finding resources beyond covered services
• Communicate with family and providers to document progress and outcomes
• Manage caseload and maintain documentation per agency standards
• Provide Covered Behavioral Health services per BHSP and service provisions
• Perform other duties as assigned
• De-escalate clients using approved crisis intervention methods
