Job summary

Copa Health is seeking an ACT Rehabilitation Specialist to join our Assertive Community Treatment team. The ACT Rehab specialist helps members obtain and maintain involvement in employment and community activities, collaborating with the clinical team to deliver rehabilitation skills training as specified in each consumer’s service plan.

Qualifications

• HS Diploma/GED with 6 years of full-time direct care experience.
• Bachelor’s Degree in a related field or a non-related field with 2 years experience preferred.
• 1 year of vocational rehabilitation training/experience preferred.
• Fingerprint clearance card issued by AZ DPS or eligibility within 8 weeks of hire.
• Reliable transportation and AZ Driver’s License with insurance.
• TB Test clearance required at time of hire.

Responsibilities

• Facilitate recovery and support achievement of service plan goals.
• Provide supportive counseling and skills development in community settings.
• Coordinate with Prescribing Clinician, Nurse and team to implement service plans.
• Attend inter/intra-agency staffing and maintain crisis plans as needed.
• Assist families in accessing services and entitlements.
• Transport members to required services as outlined in their plan.
