Family Health Services Corporation seeks an ACT-based vocational specialist to assist patients in identifying strengths and pursuing education or employment goals. You will provide engagement, assessments, job development, and ongoing support within the ACT framework.

You will educate patients about disability benefits and work with local businesses to secure employment placements, while maintaining strong case management and collaboration with the ACT team in diverse settings.

Formación

• Bachelor’s degree in human services or related field required.
• Experience as vocational specialist or CBRS preferred.
• Experience with SPMI populations and recovery-oriented services.

Responsabilidades

• Assist patients in identifying strengths and goals for recovery and employment.
• Provide engagement, vocational assessment, job development and placement.
• Educate patients on disability benefits relevant to work goals.
• Develop relationships with local businesses for employment placements.
• Maintain case files and referrals; coordinate with ACT team.
