Job summary

Catholic Charities of the Archdiocese of Washington is hiring a Peer Specialist to provide Assertive Community Treatment (ACT) services in the community. You will support ACT teams in delivering highly individualized care aligned with the ACT Fidelity Model.

Responsibilities include teaching ADLs, offering one-on-one support in offices and communities, and helping clients obtain benefits such as Medicaid and housing.

Qualifications

  • High school diploma or equivalency.
  • Peer Specialist certification from DC DBH is required.
  • 2 years' experience in a social services environment.

Responsibilities

  • Provide Assertive Community Treatment (ACT) services to consumers.
  • Teach Activities of Daily Living (ADL) including personal hygiene, grooming and self-care, and related tasks.
  • Provide one-on-one office and community-based support for ACT consumers for six months.
  • Travel to and from appointments to support consumers.
  • Assist consumers with Medicaid, Social Security, housing and other benefits.
  • Foster coping and community integration skills and support family relationships.
  • Document all services in the electronic database and review progress notes weekly.
  • Attend supervisory sessions and clinical rounds with the team.
  • Maintain adherence to ACT Fidelity Model and organizational policies.
