Job summary

Carolina Outreach seeks a full-time Peer Support Professional to join our Community Support Team in Raleigh, NC. You will provide clinical services including tenancy support, service coordination, and counseling to individuals on the CST program, using Permanent Supportive Housing models and CBT/MI approaches.

Responsibilities include documenting treatment plans, collaborating with care teams, and responding to client needs around the clock. A valid driver’s license and clean record are required.

Qualifications

• Certification as a Peer Support Specialist and at least two years of sustained recovery from mental health or substance use.
• Must obtain NVCI certification and have training in the population served and client rights/confidentiality.
• Must have a valid driver’s license and current automobile insurance.

Responsibilities

• Identify, plan, schedule, and implement services for the client.
• Develop transition plans when clients move in and out of services.
• Advocate in the community.
• Provide written documentation including treatment notes, goals, and plans for the medical record.
• Respond to clients and co-workers 24/7/365 as needed.
• Be free from all illicit substance use.
