Job summary

Terros Health in Tempe, AZ is seeking an ACT Housing Specialist to join our interdisciplinary ACT clinical team serving individuals with Serious Mental Illness under Court Ordered Treatment at our Priest Recovery Health Center.

You will provide direct treatment, housing assistance, and tenancy support, coordinating with the ACT team to locate affordable housing and develop individual service plans; on-call crisis intervention and weekend shifts are required.

Qualifications

3 years experience in the delivery of behavioral health.
Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience, or
Master's Degree in Behavioral Health field.
3 years housing specialist experience preferred; minimum 1 year required.
Must have a good understanding of housing resources within Maricopa County.

Responsibilities

Provides direct treatment, rehabilitative and supportive services that support independent living in a community setting.
Assists patient in locating housing of their choice and access to affordable housing.
Supports patients in protecting tenancy rights regardless of ACT participation.
Develops individual service plans and acts as patient advocate.
Provides on-call crisis intervention and weekend/holiday coverage.
