Job summary

Terros Health's Priest Recovery Health Center in Tempe, AZ seeks an ACT Housing Specialist to join our interdisciplinary ACT clinical team serving individuals with SMI or mental health diagnoses under Court Ordered Treatment. The role provides direct treatment and supports patients in locating housing, protecting tenancy rights, and coordinating with families and community agencies.

Schedule is Tue–Sat, 7am–4pm with flexibility for weekends and holidays.

Qualifications

• Direct treatment, rehabilitative and supportive services to support independent living in a community setting.
• Assess patient needs and help locate housing options, access affordable, safe, decent, and permanent housing.
• Advocate for patients and liaise with families and community agencies.
• Provide on-call crisis intervention services and weekend/holiday coverage.

• Participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies.
• Provides on-call crisis intervention services, as well as weekend and holiday coverage. (schedule flexibility required)

Seeking an ACT Housing Specialist to join our interdisciplinary ACT clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Recovery Health Center in Tempe, AZ.

Schedule: Tues - Sat; 7am - 4pm

Must have very flexible schedule as the team provides treatment 7 days/week, including weekends and holidays
