Job summary

Terros Health in Tempe, AZ is seeking an ACT Housing Specialist to join our interdisciplinary ACT clinical team serving individuals with Serious Mental Illness under Court Ordered Treatment at our Priest Recovery Health Center.

You will provide direct treatment, housing assistance, and tenancy support, coordinating with the ACT team to locate affordable housing and develop individual service plans; on-call crisis intervention and weekend shifts are required.

Qualifications

• 3 years experience in the delivery of behavioral health.
• Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience, or
• Master's Degree in Behavioral Health field.
• 3 years housing specialist experience preferred; minimum 1 year required.
• Must have a good understanding of housing resources within Maricopa County.

Responsibilities

• Provides direct treatment, rehabilitative and supportive services that support independent living in a community setting.
• Assists patient in locating housing of their choice and access to affordable housing.
• Supports patients in protecting tenancy rights regardless of ACT participation.
• Develops individual service plans and acts as patient advocate.
• Provides on-call crisis intervention and weekend/holiday coverage.

Additional Language Differential Pay Available.

Schedule: Tues - Sat; 7am - 4pm

Must have very flexible schedule as the team provides treatment 7 days/week, including weekends and holidays

Generous PTO/Sick Time (4+ weeks in year 1)

IN ADDITION - YOU RECEIVE $3/hour ACT Differential, plus on-call stipend

Duties include:

• Provides direct treatment, rehabilitative and supportive services that support independent living in a community setting to members.
• Works in conjunction with the patient and ACT team staff to assess patient needs and wants, assist the patient in locating housing of their choice, assists in accessing affordable, safe, decent, and permanent housing.
• Supports patients in protecting their tenancy rights regardless of participation in ACT team services.
• Participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies.
• Provides on-call crisis intervention services, as well as weekend and holiday coverage. (schedule flexibility required)

QualificationsQualifications:

• 3 years experience in the delivery of behavioral health, or
• Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience, or
• Master's Degree in Behavioral Health field
• 3 years working as a housing specialist preferred - minimum of 1 year required
• Must have a good understanding of housing resources within Maricopa County
• Must be 21 years of age or older, with a valid Arizona driver’s license, and meet requirements of Terros Health’s driving policy
• Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for one within 7 working days of employment.
• Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
• Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
• Must pass a TB test, background check, and other pre-employment screenings.
• Must pass a 25 wpm typing test.
