Job summary

Southwest Network is seeking a housing specialist to join the Assertive Community Treatment (ACT) team. You will assess housing and daily living needs, and lead in service planning for adults in a community setting.

You will coordinate services, develop individualized ISP plans, provide after-hours support, and deliver direct housing-related care. A sign-on bonus is available and benefits are included.

Qualifications

• Master’s or bachelor’s degree in behavioral health related field, or equivalent experience.
• One year related experience for non-related bachelor’s degree.
• One year related experience for an associate degree.
• Three years related experience with a high school diploma.

Responsibilities

• Assess housing, residential and activities of daily living skills and needs.
• Lead service planning for housing support services.
• Coordinate, monitor and provide direct services in a community setting.

As a member of an Assertive Community Treatment (ACT) team, functions as the housing specialist with primary responsibility for the provision of housing support services in accordance with SAMHSA’s evidence-based practice of permanent supportive housing. Provides comprehensive assessment of housing, residential and activities of daily living skills and needs, and leads in service planning for adult daily living skills and housing support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.

EDUCATION and/or EXPERIENCE, LICENSURE and CERTIFICATION

• Master’s or bachelor’s degree in a field related or similar to behavioral health; a bachelor’s degree in a non-related field and one year related experience; an associate degree in any field and one year related experience; or a high school diploma or equivalency certificate and three years related experiece
• Minimum one year working with seriously mentally ill adults required; previous experience working on an ACT team or equivalent experience preferred
• Minimum one (1) year experience working with behavioral health residential programs and/or housing-related services and resources required
• Must have and maintain a valid Arizona driver’s license
• Must be at least 21 years of age
