Job summary

Southwest Network, Inc. is seeking a Housing Specialist to join an Assertive Community Treatment (ACT) team. The role centers on housing support, comprehensive assessment, ISP planning, and community-based service coordination to promote independent living for adults.

Qualifications include a degree in behavioral health and at least one year with seriously mentally ill adults; Arizona driver’s license is required. The position includes after-hours coverage and a competitive benefits package.

Qualifications

• Master’s or bachelor’s degree in behavioral health or related field.
• Minimum one year working with seriously mentally ill adults.
• Experience with ACT team or equivalent preferred.
• Must have and maintain a valid Arizona driver’s license and be at least 21.

Responsibilities

• Provide housing support services per SAMHSA evidence-based practice for permanent supportive housing.
• Assess housing, daily living skills, and plan ISPs.
• Coordinate and monitor services in community settings; provide direct services.
• Provide after-hours care and on-call coverage to all members.
