Community Partners, NH seeks an ACT Functional Support Specialist to deliver recovery‑focused community and home‑based counseling and therapeutic services to adults with serious mental illness. You will link clients to community resources and coordinate with local services to support recovery.

The role requires a Bachelor's in Human Services or related field, and a driver’s license with vehicle liability coverage.

• Experience with adults with mental health issues preferred.
• Excellent oral and written communication skills expected.
• Must hold a valid driver’s license and have vehicle liability coverage.

The Role:

As a ACT Functional Support Specialist, you will be providing recovery focused community and home-based supportive counseling and therapeutic services, as well as linkage to and consultation with community resources for adults with serious mental illness.

What we’re looking for:

• Bachelor’s Degree in Human Services or related field such as Social Work, Psychology or Education is required
• Experience with adults with mental health issues and their families preferred
• Excellent oral and written communication skills
• Ability to utilize technology provided to perform requirements of the job
• Hold a valid driver’s license and proof of vehicle liability coverage.
