Qualifications

• Bachelor's degree in psychology or social work.
• Preference for 1–2 years of prior experience.
• Strong written and verbal communication skills.
• Valid driver's license with a clean driving record.
• CPR/First Aid training recommended; Defensive Driving training available but not required.

Responsibilities

• Provide Functional Support Services (FSS) to eligible adults and their families in community settings.
• Deliver Targeted Case Management (TCM) services to help adults access medical, social, educational and other services.
• Provide Illness Management and Recovery (IMR) services, individual or group.
• Collaborate with Team Leader to establish team recovery goals and ensure client outcomes.
• Maintain timely clinical documentation and meet required paperwork standards.
• Build and monitor a natural support system with family, friends, and community resources.
• Occasionally adjust schedule to ensure continuity of care and client support.
