Job summary

Heritage Behavioral Health Center in Decatur, IL, is seeking a Community Support Specialist for the Assertive Community Treatment (ACT) team. You will deliver intensive, community-based mental health services to adults with serious mental illness in homes, shelters, and other community settings.

You will work with a multidisciplinary team to prevent hospitalization, build life skills, and support recovery goals.

Qualifications

• High School Diploma or GED plus 5 years of supervised mental health/social service experience.
• Bachelor’s degree in social work, psychology, education, or human services preferred.
• Ability to work in a community-based ACT setting and coordinate with a multidisciplinary team.

Responsibilities

• Provide intensive, community-based mental health services to adults with serious mental illness.
• Engage clients by phone and in-person in community settings to support stability and recovery.
• Complete comprehensive assessments covering mental health, substance use, housing, employment, and transportation.
• Coordinate referrals to primary care and behavioral health services.
• Document services and client progress promptly and accurately.
