Prairie Ridge Integrated Behavioral Healthcare is expanding its Assertive Community Treatment (ACT) team in Marshalltown. You’ll deliver community-based treatment and practical support to adults with serious mental illness, helping them achieve stability and independence.

You will work with a multidisciplinary team to develop personalized goals, coordinate services, and advocate for clients while maintaining confidentiality and professional standards.

Formación

• Bachelor’s degree in social work, human services, sociology, psychology or education or related field.
• Two years of community-based rehabilitation, behavioral health, or related human services experience preferred.
• Ability to work with adults with serious and persistent mental illness and make decisions accordingly.
• Familiarity with psychotropic medications and their side effects.
• Excellent communication, problem-solving and crisis mediation skills.
• Valid driver’s license, proof of insurance and ability to travel for organization business.

Responsabilidades

• Develop an Integrated Treatment Team Plan (ITTP) and implement Individual Program Goals.
• Provide direct, face-to-face, community-based services in homes and community settings.
• Transport clients as needed to support treatment plan goals and access to services.
• Develop crisis plans and respond to psychiatric crises with the team.
• Monitor client progress and update individual service goals.
• Coordinate activities with the Treatment Team and participate in on-call rotation.
• Maintain timely documentation of services and client progress.
• Advocate for clients and coordinate resources with other providers.
• Assist with discharge planning and ensure adherence to confidentiality requirements.
