Aurora Mental Health & Recovery is hiring a full-time ACT Case Manager in Aurora, CO. You will provide direct client support, coordinate care, and assist with benefits processing, within an onsite, Monday–Friday work schedule.

Formación

• Bachelor’s degree as listed or equivalent behavioral health experience.
• Strong understanding of behavioral health services and community resources.
• Ability to document and coordinate care efficiently.

Responsabilidades

• Provide direct client contact and coordinate care for ACT participants.
• Assist clients with applications for income, medical, housing, or other benefits.
• Arrange transportation to access needed services and resources.
• Deliver comprehensive psychosocial services to improve functioning.
• Help clients set goals and plan for appropriate services.
• Instruct clients in daily living skills, medications, budgeting, and safety.
• Support clients in finding employment opportunities and vocational programs.
• Educate families and community partners to maximize benefits.

Bachelor's degree in psychology, social work or related human services degree
