Job summary

Aurora Mental Health & Recovery (AMHR) is hiring a full-time ACT Case Manager to provide direct client contact, coordinate care, and manage funds for adults with serious mental illness. This on-site role emphasizes psychosocial support, care coordination, and helping clients navigate community resources.

The position requires a Bachelor's degree in a human services field, a current Colorado driver's license, and a compassionate, detail-oriented approach.

Qualifications

• Bachelor's degree in psychology, social work or related human services field or equivalent experience.
• Valid Colorado Driver's License with acceptable driving record.
• Interest in behavioral health field and ability to assess crises.

Responsibilities

• Assist clients with basic needs and benefits applications.
• Coordinate transportation and access to services.
• Provide psychosocial services to improve functioning.
• Guide clients in goal setting and planning for services.
• Teach daily living skills and medication management where applicable.
• Support clients in finding employment or vocational resources.
• Educate families and others to maximize benefits and minimize problems.
