Job summary

Aurora Mental Health & Recovery in Aurora, CO is seeking a full-time Case Manager for our Assertive Community Treatment program. This on-site role supports adults with serious mental illness, coordinating care and services in the client’s environment.

You will participate in assessments, coordinate SSI/SSA payee funds, and work with an interdisciplinary team to promote recovery and community integration. Strong communication and organization are essential.

Qualifications

• Bachelor’s degree in psychology, social work or related human services field.
• Or Bachelor’s Degree in unrelated field with at least one year of behavioral health experience.
• Valid Colorado Driver’s License with acceptable driving record.
• Demonstrated interest in behavioral health.

Responsibilities

• Coordinate care and ensure access to services for clients in ACT.
• Assist clients with applications for income, medical, housing, or other benefits.
• Arrange transportation to access needed services.
• Provide psychosocial services to improve client functioning.
• Guide clients in daily living skills and medication use.
• Support clients in finding employment or vocational opportunities.
• Educate and support families and community partners to maximize benefits.
• Identify natural support systems to promote treatment engagement.
• Educate clients about available community services.
