Job summary

Lincoln County Behavioral Health is seeking a Case Manager II or III to join the A.C.T. program. The role provides case management, care coordination, advocacy, and symptom-management support for clients with mental health needs.

Responsibilities include intake screens, assessment and planning under QMHP supervision, and linkage to community resources. Strong documentation and collaboration with QMHPs required.

Qualifications

• CM2 requires QMHA certification or eligibility within 6 months.
• CM3 requires Bachelor’s in behavioral science.
• Oregon Driver’s License within 30 days.

Responsibilities

• Conduct intake screens and assessments related to skill building or case management.
• Perform assessment and treatment planning under direct supervision of a QMHP.
• Assist in individual, family or group therapy and co-facilitate psychoeducation groups.
• Provide care coordination, advocacy, and community referrals.
• Complete records and reports per agency standards and Oregon rules.
• Provide aftercare planning and follow-up services.
• Collect appropriate laboratory samples as indicated.
