Job summary

Qualifications

• Psychosocial assessments and treatment planning experience with serious mental illness.
• Experience coordinating services and resources in the community.
• Ability to advocate for clients and document clearly and professionally.
• Fluent written and spoken communication; intercultural sensitivity.

Responsibilities

• Provide psychosocial assessments and treatment planning.
• Deliver case management and service coordination.
• Provide skills training and crisis intervention.
• Support symptom management for individuals with serious mental illness.
• Collaborate with ACT team to staff individuals served daily.
• Coordinate referrals and access to medical, social, vocational, and financial supports.
• Maintain professional boundaries and adhere to trauma-informed, person-centered care.

Education

Experience in Texas Recovery and Resiliency (TRR) preferred.

Knowledge of Trauma-Informed Care Practices preferred

At least two years of experience in working with the community, preferred.

Must have reliable transportation and a valid driver’s license.
