Qualifikationen

• CM2 requires a Bachelor’s in behavioral science (or related field); QMHA certification preferred or eligible within 6 months.
• CM3 requires a Bachelor's in behavioral science and 2–3 years of clinical/treatment experience in behavioral health.
• Oregon driver’s license within 30 days of appointment.

Aufgaben

• Provide case management and care coordination to behavioral health clients in the ACT program.
• Assist with intake screens, assessments, and treatment planning under QMHP supervision.
• Support individual, family, and group psychoeducation; facilitate education modules.
• Coordinate referrals, advocacy, and access to community resources.
• Complete records per agency standards and Oregon rules; provide aftercare planning and follow-up.
