Mental Health Resources Inc. seeks an Assertive Community Treatment (ACT) Case Manager in Minneapolis, MN. The role supports individuals with serious mental illness and aims to prevent hospitalization through community-based services.

A 4-day work week option may be available. The position offers a salary range of $58,656 to $66,456 yearly, with language bonuses for Spanish, Hmong, or Somali proficiency and comprehensive benefits.

Qualifications

• Spanish, Hmong and Somali language skills are valued and may lead to a salary bonus.
• Experience in community-based mental health and case management is preferred.

Responsibilities

• Develop individualized plans of services to promote independence in the community.
• Monitor medication adherence and adjust plans as needed.
• Collaborate with family and significant others to support recovery.
• Advocate for client rights, privacy and understanding of procedures.
• Treat clients with dignity and cultural sensitivity; ensure timely communications.
• Maintain continuity of care and coordinate with the ACT team; cover when needed.
• Attend staff meetings and trainings; uphold confidentiality and documentation standards.
• Ensure mandated processes are followed and meet productivity expectations.
• Maintain up-to-date chart information, ROIs, and contacts.
