Wellvance Community Mental Health Authority is seeking an Assertive Community Treatment (ACT) Case Manager in Michigan. You will work within the ACT team to help individuals with severe mental illness manage symptoms, improve daily functioning, and integrate into community life.

The role requires a bachelor’s degree in a human services field, at least one year of mental health experience, and certification such as QMHP/QIDP/CMHP or Michigan licensure.

Qualifications

• Bachelor's degree in a human services field.
• One year of experience in the mental health field.
• Certification as QMHP, QIDP, CMHP, or Michigan licensure in your field.

Responsibilities

• Help individuals with severe mental illness manage symptoms and achieve quality of life.
• Coordinate services and travel within the service area as part of the ACT team.
• Follow ACT fidelity model and Wellvance policies.

Education

Bachelor's degree in a human services field

Michigan licensure or certification in field (QMHP/QIDP/CMHP)

1 year mental health experience

Valid Michigan driver's license
