Mental Health Resources, Inc. is seeking a Case Manager in Saint Paul, MN to coordinate services and support individuals recovering from mental illness. You will work with clients to create plans that enable independent living and family involvement as needed.

Role focuses on recovery, collaboration with families, and culturally respectful care, with ongoing supervision and opportunities for professional development within a supportive team.

Formación

• Bachelor's degree in social work, psychology or related field.
• At least 2000 hours of supervised mental health experience with adults.
• Knowledge of local community resources to benefit clients.
• Without a bachelor's degree: RN with 3+ years specialized training in psychiatry or 3+ years combined community work.

Responsabilidades

• Coordinate integrated services and supports toward client recovery.
• Develop Individual Community/Family Plans for independence and well‑being.
• Educate and collaborate with significant others and family members.
• Treat clients with dignity, applying culturally sensitive care.
• Ensure services align with plans and are continuously monitored.
• Document changes in client status and needs.
• Participate in staff meetings and trainings.
