Mental Health Resources, Inc. is hiring a Full Time Professional in Saint Paul, MN to serve as a Targeted Case Manager. The role focuses on coordinating services to help adults recover and stay out of hospitals through individualized plans and community resources.

You will work primarily Monday–Friday with an option for a 4-day work week. The position emphasizes patient-centered care, collaboration with families, and ongoing documentation of progress in the care plan.

Formación

• Bachelor’s degree in a social services field and eligibility for supervision/continuing education.
• Minimum 2000 hours of supervised mental health service experience with adults.
• Knowledgeable about local community resources and how to access them.
• If no bachelor’s degree, alternative pathways include nursing with relevant experience and training.

Responsabilidades

• Coordinate integrated services to support each client’s recovery goals.
• Develop Individual Community or Family Plans with services and supports for independence.
• Educate and collaborate with family members to assist recovery efforts.
• Document changes in client status and needs and ensure continuity of care.
• Attend staff meetings and trainings as part of ongoing professional development.
