Qualifications

• Master's degree in a human services related field.
• Bachelor's degree with 1 year relevant experience preferred.
• High School Diploma or GED with at least 3 years experience.
• Certified Peer Specialist (CPS) or Certified Family Specialist (CFS).
• Valid driver's license.

Responsibilities

• Coordinate comprehensive treatment/service plans for ACT participants.
• Provide case management, education, and skills coaching.
• Assist individuals with access to housing, benefits, and transportation.
• Support primary care and other specialist access as needed.
• Organize group/social activities to build skills.
• Provide on-call crisis intervention as scheduled.
• Engage families and crisis support as required.
• Document services in a timely, compliant manner.
