Job summary

Hackensack Meridian Health is seeking a Transition Assistant, Accountable Care Organization to support quality metrics and patient transitions within the ACO framework. You will collaborate with care coordinators, physicians, and staff to identify gaps and improve care delivery in New Jersey.

The role includes auditing data, training providers on payer program metrics, and developing workflows that align with CMS guidelines and ACO standards, while communicating findings in monthly reports.

Qualifications

• Audit and assess data to identify gaps in quality metrics.
• Train staff on payer program metrics and CMS guidelines.
• Support workflow design to improve clinical quality documentation.
• Prepare and present monthly quality reports to practices.
• Assist in coordinating safe patient transitions across care settings.
• Demonstrate strong organizational and communication skills.

Responsibilities

• Audit clinical records for gaps in quality care metrics.
• Train, educate and update ACO physicians and staff on payer program metrics.
• Assist in workflow design that promotes compliance to ACO standards and clinical quality metrics.
• Provide monthly reports to practice to assist with quality metric performance.
• Work with providers, care coordinators and office staff to ensure safe patient transitions.
• Travel to assigned ACO offices to ensure compliance with ACO policies and procedures.
• Identify the needs of the patient population served and adjust care accordingly.
