Job summary

Lightshare Behavioral Wellness & Recovery is seeking an Access Navigator to guide individuals through complex systems and ensure timely access to services. You will assess needs, identify barriers, and coordinate with internal teams and external partners to streamline access and referrals.

Strong communication, organization, and problem-solving are essential, along with a customer-focused approach to equity.

Qualifications

• High school diploma or equivalent; associate or bachelor’s degree preferred.
• Experience in case management, social services, healthcare navigation, or related field.
• Strong communication and interpersonal skills.
• Ability to manage multiple tasks and prioritize effectively.
• Proficiency with basic computer applications and data entry.

Responsibilities

• Assess client needs and eligibility for programs and services.
• Provide information and guidance on resources and access procedures.
• Coordinate with internal teams and external partners to deliver services.
• Maintain records of client interactions, referrals, and outcomes.
• Advocate to overcome barriers and ensure equitable access.
• Monitor client progress and follow up to ensure successful navigation.
• Participate in process improvements to enhance access.
