Job summary

Planned Parenthood of the Pacific Southwest is seeking a Patient Navigator to provide field-based case management and care coordination for patients seeking abortion and reproductive health services. You will work with the Abortion Access team to remove barriers, assist with insurance enrollment, and connect patients to financial resources and community supports.

The role emphasizes navigating complex health systems, ensuring timely access to care, and educating patients on benefits and coverage

Qualifications

• Associates Degree preferred.
• 1 year experience of patient navigation.
• Spanish language preference.

Responsibilities

• Coordinate enrollment in public and private insurance programs and assist with applications.
• Provide financial advocacy and medication access assistance for patients.
• Serve as a main contact for abortion and reproductive health navigation and care coordination.
• Support prior authorization, referrals, and follow-up to ensure timely access to services.
• Educate patients about insurance benefits, coverage limits, and provider networks.
