Job summary

Planned Parenthood of Delaware is seeking a Patient and Community Navigator to bridge clinical care and community needs. You will conduct proactive outreach, coordinate appointments, resolve barriers to access, and engage community advisory groups to improve equity in health services.

Qualifications

• High School diploma or equivalency required; associate degree preferred.
• Minimum 1 year in patient navigation, care coordination, or similar role; experience serving marginalized communities preferred.
• Experience with electronic health records and practice management systems; Epic experience preferred.

Responsibilities

• Conduct proactive outreach to patients due/overdue for services via phone, text, and electronic platforms.
• Organize and manage appointments aligning with clinical needs and patient preferences.
• Identify and resolve barriers to care, including transportation, language access, and social determinants of health.
