Job summary

The People Concern is seeking a Case Manager Apprentice to join the Permanent Housing Services team for a five-week program focused on learning and skill-building in case management for clients experiencing homelessness. You will participate in educational sessions and site-based shadowing to develop skills in housing placement, self-sufficiency, and community integration.

Qualifications

• Valid driver’s license, reliable car, auto insurance, and an acceptable driving record.
• Willing and able to transport Clients in your personal vehicle.
• Demonstrated knowledge of issues faced by a culturally diverse population.
• Understanding of Outlook (email/calendar)

Responsibilities

• Collaborate with case management staff to build relationships with Clients and promote independence and health.
• Assist in intake and enrollment of Clients including review of program consents and subsidy applications.
• Support biopsychosocial and functional assessments to guide individualized planning.
• Assist Clients in applying for and obtaining benefits to which they are entitled.
• Coordinate appointments, transportation, and follow-up services for Clients to improve access to health care and community resources.
• Observe and model life skills such as budgeting, housekeeping, meal prep, appointment scheduling, and health maintenance.
• Review time management strategies to balance client engagement with administrative tasks.
• Participate in weekly trainings, staff meetings, supervision, and evaluations.
• Contribute to a work environment emphasizing client choice, harm reduction, trauma-informed care, and housing-first principles.
• Engage in crisis assessment training and de-escalation practice.
• Build field safety skills for home visits and transportation.
• Practice writing progress notes in electronic databases.
• Review charts as guided by agency expectations.
• Maintain client confidentiality as per HIPAA and internal guidelines.
• Communicate effectively with Apprenticeship staff and collaborators.
