Job summary

A valid RN license, BLS, and willingness to travel are required; malpractice insurance and background checks apply.

Qualifications

• RN license active in state of practice.
• Willingness to travel to patients' homes.
• Graduation from an accredited school of nursing.
• Current BLS Certification.
• Ability to pass a state & federal background check.
• Malpractice Insurance is required.
• Strong problem-solving and empathetic approach.

Responsibilities

• Conduct thorough patient assessments and provide personalized in-home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) with dignity and respect.
• Coordinate schedule with patients according to plan of care and preferences.
• Monitor symptoms and promptly report changes to the care team.
• Administer medications and treatments as prescribed.
• Communicate with Case Managers and physicians to ensure unified care.
• Document patient information accurately in the EMR.
• Provide emotional support to patients and families.
