Job summary

Fusion Medical Staffing is seeking a travel Physical Therapist for a 13-week assignment in Elmwood Park, IL. You will evaluate and treat patients, develop personalized plans, and collaborate with the clinical team while maintaining documentation and safety standards.

Qualifications

• Valid Physical Therapy license in Illinois
• Current BLS certification (AHA/ARC)
• Doctorate degree in physical therapy (preferred)

Responsibilities

• Evaluate patients’ physical conditions using exams and history to identify needs.
• Develop personalized rehabilitation programs based on assessment findings and goals.
• Implement treatment plans to improve mobility and reduce pain.
• Document evaluations, interventions, and outcomes in medical records.
• Collaborate with physicians and other healthcare professionals for coordinated care.
• Educate patients/caregivers on home programs and prevention strategies.
• Maintain a safe treatment environment adhering to infection control and safety standards.
