Qualifications
- Current and active LPN license.
- Experience in skilled nursing or long-term care preferred.
- Strong clinical and nursing skills.
- Compassion for elderly residents.
- Strong communication and interpersonal skills.
- Reliable attendance and dependability.
- Team-oriented attitude.
- Commitment to high-quality resident care.
- Ability to work assigned shifts and weekend rotations.
Responsibilities
- Provide direct nursing care to residents.
- Administer medications and treatments as ordered.
- Monitor residents and identify changes in condition.
- Communicate changes in resident condition to appropriate clinical staff.
- Complete nursing documentation accurately and timely.
- Follow individualized resident care plans.
- Work closely with RNs, CNAs, physicians, therapists, and other team members.
- Assist with admissions, transfers, and discharges as assigned.
- Maintain resident safety, dignity, and comfort.
- Communicate professionally with residents and family members.
- Follow facility policies and applicable nursing standards.
- Deliver compassionate, high-quality, resident-centered care.
