Job summary

Hallway Healthcare in Manteca, CA seeks a Certified Nursing Assistant to provide 1:1 in-home care and assist with daily living activities. You will monitor vital signs, document in Alayacare, and report changes to a nurse supervisor. Requires 6 months of experience and EMR literacy.

Qualifications

• 6 months of experience in home health or similar setting.
• Computer literate with EMR experience.

Responsibilities

• Complete documentation on Alayacare by the end of each shift.
• Provide direct patient care under the supervision of a registered nurse or licensed practical nurse.
• Assist with activities of daily living such as bathing, dressing, cooking, feeding and light housekeeping.
• Monitor and record vital signs and report changes to nursing staff.
• Assist with patient mobility and transfers.
• Transport patients to appointments or errands.
• Provide emotional support to patients and their families.
• Maintain a clean and safe home environment.
