Job summary

Hallway Healthcare is seeking a Certified Nursing Assistant to join our in-home care team in Dublin, CA. This role involves delivering direct patient care, assisting with daily living activities, and reporting health changes to the supervising nurse.

Shifts are 8am–8pm or 8pm–8am; we typically assign one patient per shift to provide focused, compassionate care. 6 months of experience and EMR familiarity are preferred; valid driver’s license required.

Qualifications

• Minimum 6 months of experience in home care or nursing support.
• Computer literacy with EMR experience.
• Valid driver’s license required.

Responsibilities

• Complete documentation on Alayacare by the end of each shift.
• Provide direct patient care under the supervision of a registered nurse or licensed practical nurse.
• Assist with activities of daily living, such as bathing, dressing, cooking, feeding and light housekeeping.
• Monitor and record vital signs, such as blood pressure, temperature, and pulse.
• Assist with patient assessments and report any changes in condition to the nursing staff.
• Assist with patient mobility and transfers.
• Transport patients to doctors appointments, grocery stores or pharmacies.
• Provide emotional support to patients and their families.
• Maintain a clean and safe environment for patients.
